- Select the form of your region from the map above.
- Fill the online form.
- Click on the box that says "I accept the terms and conditions" on the " Terms & Conditions" page.
- Either save the form on your computer for later use OR print it. After you print the filled form, put your signature and thumb impression where it says "Customer's Signature".
- Scan the form.
- Attach and submit scanned copy of the form below.

Following will also be required for service activation:

• Undertaking on your company's letterhead (if you are applying on behalf of your company)
• Legible copy of your CNIC
• NTN (if you ar applying on behalf of your company)
• Call Center Certificate (if your company is a call center)
• PSEB certificate (if your company is a software house)
• Payment (our sales agent will contact you after you send the documents)
